A Simple 4-Step Admissions Process

  1. Initial Inquiry & Consultation

    Contact our admissions department. We discuss your loved one's medical needs, desired services, and answer initial questions about our facility and availability.

  2. Clinical Assessment

    Our clinical team reviews medical records and performs an assessment (often at the hospital or home) to ensure we can meet all care requirements.

  3. Insurance Verification

    We verify Medicare, Medicaid, or private insurance coverage and discuss payment options, financial responsibility, and provide a transparent cost overview.

  4. Move-In Coordination

    Once approved, we finalize the admission paperwork, coordinate arrival logistics with the hospital discharge planner or family, and welcome the new resident.

"What to Bring" Checklist

To make the transition as comfortable as possible, we recommend packing the following. Please label all personal belongings.

  • 5–7 days of comfortable, loose-fitting clothing.
  • Non-slip sturdy shoes (sneakers preferred) and socks.
  • Nightwear (pajamas, robe, slippers).
  • Personal toiletries (toothbrush, comb, preferred lotions, etc.).
  • Labeled glasses, hearing aids, and dentures.
  • A few framed family photos or small personal mementos.
  • Please do not bring jewelry, large amounts of cash, or valuables.
